You can submit the following documentation to your Asepeyo healthcare centre. In each case, the person who must submit the documentation is indicated:
In all cases
- Application for direct payment due to a temporary disability: mutual insurance company
- Valid DNI (photocopy): Interested
- Medical leave report, confirmation reports and fit to work report (photocopy). Only for common contingencies: the interested party
- Original IRPF Personal Income Tax form: mutual insurance company
For temporary disability situations deriving from professional contingencies or extinction of a contract
- TC2 payroll report, payroll stub or company certificate for the month prior to the leave: company
- Updated work history report: mutual insurance company
For cases of temporary disability due to common contingencies or extinction of contract
- Updated work history report: mutual insurance company
If the work history report proves unemployment has been received in the last year:
- A certificate stating the period recognized, consumed and the regulatory monthly salary base: INEM (Spanish national employment authority)
- A certificate indicating the worker’s wage bases: Social Security Treasury
If the wage bases certificate does not cover the last 180 days of contribution, 2 of the following documents for the un-accredited period must be provided: a certificate for the last 180 days of contribution prior to the extinction of the employment relationship:
- A certificate for the last 180 days of contribution prior to the extinction of the employment relationship:
- Companies with less than 10 workers: company
- Companies with 10 or more workers: Spanish Public Employment Service
- Photocopies of the TC2 payroll report forms showing contributions for the last 180 days of contribution prior to the extinction of the employment relationship or payroll stubs for the same period: company
- Payroll stubs for the last 180 days of contribution: company
For cases of dismissals or ERE (Labour Force Adjustment Plan)
- The letter of dismissal (original and photocopy): interested party
- The certificate of conciliation from the Conciliation and Arbitration Centre (CMAC) (original and photocopy): CMAC
- ERE resolution and list of affected parties: company
